Yes-you own your policy and can keep it until age 99. In order to continue your policy, a continuation packet must be completed and returned to our office within 60 days of your last paycheck. After 60 days, your policy is terminated and cannot be reinstated.
Services
No medical form necessary for new hires within the first 60 days of employment (for an amount up to 4x your annual salary with a maximum of $300,000.)
Once 60 days have passed, a short medical questionnaire is required in the policy application.
This medical questionnaire will also be required in order to increase Employee and/or Spouse/Domestic Partner coverage amount.
Employee and Spouse/Domestic Partner (Sp/Dp) rates are the same in accordance with your respective ages.
The maximum amount of coverage an employee may apply for is ten times your salary, not to exceed $750,000 (minimum amount is $10,000.)
The maximum Sp/Dp may apply for is $380,000.
The Employee must carry coverage in order to apply for Sp/Dp Coverage and Accidental Death & Dismemberment (AD&D) coverage.
AD&D Life Insurance can be added to base coverage up to $750,000 regardless of salary (monthly rates are listed on Application Form.)

If you are going to take a leave of absence from work, you will need to contact Western Insurance directly to set up direct monthly payments to continue your coverage.
